Cody Chadwick, 32, Jamestown, ND died Friday April 20, 2018 at his home in Jamestown.

Cody was born August 2, 1985 at Jamestown, ND, the son of Mark and Shari (Opp) Chadwick. He attended school at Jamestown and received his GED in 2013.

He worked at Walmart and in 2013 he went to work at the North Dakota State Hospital where he was a Certified Med Aid.

Cody is survived by his parents, Mark Chadwick and Shari (Tom) Hoffmann; 2 half-brothers and 4 half-sisters, Trever Sahr, Miah Sahr, all of Jamestown, ND, Sara Chadwick, Mandan, ND, Lindi Chadwick, Jamie Chadwick, Levi Chadwick, all of Jamestown, ND, 1 niece, Brynlee Block and nephew, Trent Sahr, both of Jamestown.
